Registration Procedure

WHEN YOU BECOME PREGNANT
When you find out that you are pregnant, obtain a notification of pregnancy form, which is available at your local ward office/city hall. Record your name, address and the name of you hospital on the form. You will receive a Maternal and Child Health Handbook and the Mother and Child Health Bag. The Maternal and Child Health Handbook is needed for routine pregnancy health checkups. Please keep the Handbook as it can be used to certify your child’s vaccination record. The Mother and Child Health Bag contains two application forms enabling you to receive free pregnancy health checkups, as well as information on Pregnancy classes, information of health consultations for new mothers, and a birth registration form.

NOTIFICATION OF BIRTH WHEN YOUR BABY IS BORN
When your child is born, you must report the birth to the city office of the area where you reside. Ask your doctor for the certificate of live birth and submit the Birth Notification Certificate to the Registration Subsection, Resident Registration Section within 14 days of birth. City hall will issue you a certificate of registration for birth notification (Shussei Todoke Kisai Jiko Shomeisho).

WHERE TO NOTIFY
The local municipal office where the birth occurred or the municipal office where the notifying person maintains his/her current residency.

WHO SHOULD NOTIFY
Mother or Father

NECESSARY DOCUMENTS
Notification of birth (attached certificate of birth must bear the signature of the attending physician), both parents’ passports and alien registration cards, and the Mother and Child Health Handbook.

IMPORTANT NOTE
For babies born in Japan, if one parent is a Japanese national, the child automatically becomes a Japanese nationality. For other babies their nationality will be determined in accordance with the law of their parents countries.

FOREIGN RESIDENT REGISTRATION
Bring the certificate of birth notification to the Foreign Resident Registration Subsection, Resident Registration section for foreign resident registration within 60 days of birth.

APPLICATION FOR PASSPORT
Apply at your embassy in Japan.

APPLICATION FOR STATUS OF RESIDENCE AND PERIOD OF STAY
Apply at the Tokyo Regional Immigration Bureau within 30 days of birth.

REGISTRATION OF GRANTED STATUS OF RESIDENCE AND PERIOD OF STAY
Bring your baby’s passport and foreign resident registration card within 14 days of acquiring status of residence and period of stay to the Foreign Resident Registration Subsection, Resident Registration Section.
